Risk Management Information

Thank you for volunteering this season. In order to provide a safe environment for our children to enjoy the game of soccer, we ask each volunteer to complete a criminal background check.

Every adult member that has contact with or access to our players and/or their confidential data must be cleared through a national background check. This action is for the protection of not only East Pasco Youth Soccer League and its governing body, Florida Youth Soccer Association, but ultimately our most precious resource, the children entrusted to us.

Please take a moment to fill out the information on the bottom of this form and our registrar will input your name into the GotSport system. You will receive an email with your login information from GotSport which will allow you to complete the background check online.
